Living Well with Chronic Conditions (CDSMP):
This Chronic Disease Self-Management Program (CDSMP) workshop is six weeks long with one 2.5-hour session each week. There is an informational session offered prior to the first week.
This program is designed for people living with one or more on-going conditions like arthritis, heart disease, stroke, kidney disease, lung disease, depression, and many others, as well as the people who care for them. It will provide you with the tools you need to meet your health-related goals and manage your health conditions every day. It was designed to complement and enhance the care we get from our health professionals. Research shows participants are able to manage their symptoms better and communicate more easily with their doctors and loved ones.
Special topics covered in this workshop include:
- Getting a good night’s sleep
- Pain and fatigue management
- Better breathing
- Making informed treatment decisions
- Preventing falls
Diabetes Self Management Program (DSMP):
The Diabetes Self Management Program (DSMP) workshop is six weeks long with one 2.5-hour session each week. There is an informational session offered prior to the first week.
This program was designed for people or caregivers of people living with type 2 diabetes. It was designed to complement and enhance the care we get from our health professionals. You will receive a free copy of Living a Healthy Life with Chronic Conditions while focusing on Diabetes-specific workshop material.
Special topics covered in this workshop include:
- Preventing and minimizing the complications of diabetes
- Recognizing and managing hyperglycemia and hypoglycemia
- Working more effectively with health care providers
- Healthy eating and menu planning
- How to include exercise into your daily routine
- Monitoring blood sugar
Walk with Ease:
The Arthritis Foundation Walk With Ease program is an evidence-based physical activity and self-management workshop developed for people living with arthritis or other chronic health conditions who want to be more physically active.
- This program meets 2 times a week over a period of 9-weeks
- Meets in community-based small groups.
- You will receive a free copy the program book.
- Groups participate in a group discussion followed by a group walk.
Walk With Ease focuses on motivational strategies including action planning, goal setting and social support, and provides participants with the information, tips and tools they need to develop a safe exercise routine that fits their unique needs and goals.
